IMPORTANT NOTE ABOUT PDF FORMS: Information typed directly on the on-line form cannot be saved prior to closing the form/application. Do not close the form unless you have printed out a hard-copy.

Downloading Adobe Acrobat Reader

• Click here to download Adobe Acrobat Reader. This is a free plug-in.
  
• Help and information about this program is available at: http://www.adobe.com.
  

Typing on the Online Forms

• Position your cursor on the field you wish to fill in. Click once. This will position the cursor in the field. Begin typing.
  
• The fields cannot be altered and will not expand based on the amount of material you type. Please be sure information typed in the fields is visible before printing and submitting your application.
  
• You may paste text from another word processing program into the form fields. Again, field size limitations will remain in effect.
  

Once you have completed the form

• Print the form out immediately. If you close the form, all information typed will be lost.

Instructions on using the Microsoft formats (Excel or Word) of the forms instead of using the on-line PDF version forms

• To download the Microsft version of the forms, right-click on the name of the form you wish to download and select Save Target As or Download. The method for downloading the Excel forms on a Mac or PC may vary.
  
• If using the Microsft version of the forms, the Foundation requests that you do not alter the actual structure or format of the form.

I am unable to download the Excel version or to use the on-line PDF version.

• Please contact the Foundation to request hard-copy versions of the materials.
